docs: add CONTRIBUTING.md and CHANGELOG.md#9

CONTRIBUTING covers dev setup, the green-check gate, the "never log raw PII / no disk writes" rules, and PR expectations. CHANGELOG follows Keep a Changelog with a seeded 1.0.0 release and an Unreleased section. Both linked from the README. Closes #5 Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>
